Job summary

Qualifications

• Graduate of an accredited school of nursing.
• Active RN license in the state served.
• Minimum of 1 year of nursing experience.
• Exceptional critical thinking and clinical expertise with strong case management abilities.
• Commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Be a clinical leader delivering high-quality, personalized nursing care in the patient’s home.
• Craft individualized care plans and adjust them as needed.
• Collaborate with physicians, therapists, and support staff to ensure holistic care.
• Educate patients and families, providing teaching and emotional support.
• Mentor and guide LPNs, LVNs, and Home Health Aides to strengthen the care team.
• Utilize technology for AI-powered clinical documentation and best practices to optimize care delivery.
