Job summary

You will collaborate with a multidisciplinary team to ensure optimal outcomes. You will leverage your clinical expertise to educate patients and families, mentor junior staff, and navigate care delivery

Qualifications

• Graduate of an accredited school of nursing.
• Must have an active RN license in the state served.
• Minimum of 1 year of nursing experience.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Be a clinical leader delivering high-quality, personalized nursing care in a patient’s home.
• Craft individualized care by developing and adjusting comprehensive care plans.
• Collaborate with physicians, therapists, and support staff to ensure holistic care.
• Empower patients and families with essential teaching and emotional support.
• Mentor and guide LPNs, LVNs, and Home Health Aides on the care team.
• Innovate with technology, using AI-powered clinical documentation to optimize care and documentation.

No After Hours On-Call Requirements

Job Requirements

Required

• Graduate of an accredited school of nursing.

• Two years of hospital / hospice / homecare RN nursing experience preferred.
