Job summary

CHI Health at Home is part of an expansive health care network dedicated to providing better patient care with better outcomes, right at home. As a Hospice Registered Nurse, you will deliver compassionate, high-quality care, performing thorough assessments and implementing patient care plans.

You will collaborate with a multidisciplinary team to ensure optimal outcomes. You will leverage your clinical expertise to educate patients and families, mentor junior staff, and navigate care delivery

Qualifications

Graduate of an accredited school of nursing.
Must have an active RN license in the state served.
Minimum of 1 year of nursing experience.
Current CPR/BLS certification.
Valid driver’s license and reliable transportation.

Responsibilities

Be a clinical leader delivering high-quality, personalized nursing care in a patient’s home.
Craft individualized care by developing and adjusting comprehensive care plans.
Collaborate with physicians, therapists, and support staff to ensure holistic care.
Empower patients and families with essential teaching and emotional support.
Mentor and guide LPNs, LVNs, and Home Health Aides on the care team.
Innovate with technology, using AI-powered clinical documentation to optimize care and documentation.
