Job summary

CHI Health at Home is seeking a dedicated Registered Nurse to provide compassionate, patient-centered home health care. You will lead clinical care in patients' homes, developing personalized plans and coordinating with physicians, therapists, and staff to ensure holistic, high-quality outcomes.

You will mentor support staff, utilize advanced documentation tools, and advocate for patients and families as part of a mission-driven team that values dignity, equity, and innovation in every visit.

Qualifications

• Graduate of an accredited nursing program with an active RN license.
• Minimum of 1 year of nursing experience (Home Health preferred).
• Exceptional critical thinking and proven clinical expertise.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Be a clinical leader delivering high-quality, personalized nursing care in patients' homes.
• Develop, implement, and adjust comprehensive care plans to promote independence.
• Collaborate with physicians, therapists, and support staff to ensure holistic care.
• Educate patients and families, providing teaching and emotional support.
• Oversee and mentor LPNs, LVNs, and Home Health Aides to strengthen the care team.
• Utilize technology and documentation tools to optimize care delivery.
