Job summary

3HC Home Health and Hospice seeks a dedicated Hospice Social Worker (MSW) to provide emotional support and social work services to patients and families at the end of life. The role emphasizes comprehensive psychosocial assessment, care planning, and collaboration with the IDG to ensure dignified, patient-centered end-of-life care.

The position requires an MSW from a CSWE-accredited program, CPR (BCLS) certification, and preference for 1 year of health care social work experience.

Qualifications

• Master's Degree in Social Work from a CSWE-accredited school.
• One-year social work experience in a health care setting preferred.
• CPR certification (BCLS) maintained

Responsibilities

• Abides by and supports 3HC's Compliance Program and Code of Ethics.
• Creates positive experiences for patients and families, meeting expectations.
• Manages assigned cases and assists in achieving positive patient outcomes.
• Documents psychosocial assessments, plan of care, and progress notes.
• Coordinates with physicians and IDG to adjust plans of care as needed.
• Assists with bereavement support and community referrals.
• Maintains timely documentation and inter‑disciplinary communications.
• Travels to patient locations as requested by supervisor.
