Job summary

Providence at Home with Compassus is hiring a Hospice Registered Nurse to deliver person-centered care in patients’ homes in Issaquah, WA. You will assess, plan, implement, and direct nursing care with an interdisciplinary team.

The role emphasizes community health nursing, collaboration with the IDG, and on-call duties. Requires RN license; 1 year hospice/home health experience preferred.

Qualifications

• Associate or Bachelor’s degree from an accredited School of Nursing.
• Current, unencumbered RN license in the state of employment.
• 1 year of hospice or home health nursing experience preferred.

Responsibilities

• Complete patient and family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and direct the Plan of Care with the IDG.
• Regularly reevaluate needs and revise the Plan of Care as necessary.
• Initiate and revise the Plan of Care as patient status changes.
• Administer medications and treatments as prescribed by the IDG-approved Plan of Care.
• Participate in on-call duties and maintain productivity standards.
