Job summary

West Tennessee Healthcare is seeking a Registered Nurse to plan and deliver hospice home care, using the nursing process and interdisciplinary collaboration. The role requires leadership in care delegation and ongoing communication with patients, families, and the care team.

Responsibilities include initial assessments, care documentation, and adherence to professional standards; participation in performance improvement and mandatory meetings.

Qualifications

• Completion of a diploma, associate, or baccalaureate degree in nursing.
• Current RN license to practice in Tennessee; BLS certification.
• Continuing Education Units (CEUs) annually.
• Three years of nursing experience in acute, long-term care, or home care hospice.

Responsibilities

• Assess home care needs considering physical, emotional, and spiritual aspects.
• Document initial assessment and care in patient records; monitor documentation by staff.
• Provide direct care per the interdisciplinary plan of care to maintain comfort and quality of life.
• Lead and delegate care to licensed/unlicensed staff; supervise hospice aides and on-call rotation.
• Maintain confidentiality, communicate respectfully, and support patients and families through end-of-life care.
• Document patient status and teaching; keep charts up-to-date and coordinate with the physician.
• Participate in performance improvement projects and interdisciplinary team meetings.
