Job summary

West Tennessee Healthcare in Jackson, TN is seeking a Hospice Registered Nurse to plan and deliver patient care using the nursing process. You will collaborate with patients, families, and an interdisciplinary team while maintaining professional nursing standards and competency.

You will assess needs, document care, manage the care plan, supervise aides, and participate in on-call rotations and on-going improvement initiatives.

Qualifications

• Completion of a nursing diploma/ associate/ bachelor degree is required.
• Licensed as a Registered Nurse in Tennessee.
• BLS certification within orientation; CEUs annually.
• Experience in acute, long-term, home care or hospice preferred.

Responsibilities

• Assess home care needs and document assessments.
• Provide direct patient care per interdisciplinary Plan of Care.
• Manage case load and coordinate with IDT every 14 days.
• Supervise unlicensed nursing staff and hospice aides.
• Participate in on-call rotation and intake visits.
• Contribute to performance improvement projects.
