Job summary

AuthoraCare Collective in Greensboro is seeking a Hospice Home Care Case Manager (full-time, 32 hours per week across 4 days) to oversee nursing care for hospice patients in the Greensboro service area. This role requires CPR/BLS and NC RN licensure, with experience in hospice or home health being a plus.

The RN will manage the plan of care, collaborate with the interdisciplinary group, and provide education and emotional support to patients and families, including on-call coverage as scheduled.

Qualifications

• RN license in North Carolina is required.
• CPR certification from AHA or ARC Basic Life Support (BLS) required.
• Hospice/home health experience is a plus.

Responsibilities

• Manage the overall plan of care for a caseload of hospice patients within the IDG.
• Assess patient/caregiver needs, set nursing goals and interventions.
• Collaborate with physician to establish medical plan of care.
• Maintain therapeutic relationships and provide emotional support and education.
• Document outcomes and participate in on-call coverage.
