Job summary

AuthoraCare Collective is seeking a Hospice RN Home Care Case Manager for the Burlington service area. This full-time role (40 hours/week, Monday–Friday 8:00am–5:00pm) involves coordinating the overall plan of care for hospice patients, collaborating with the IDG and physicians, and providing emotional support and education to patients and caregivers.

Qualified candidates must be a graduate of an accredited School of Nursing, hold an NC RN license, and have CPR/BLS certification.

Qualifications

• Graduate of an accredited School of Nursing.
• Must be currently licensed as an RN in North Carolina.
• CPR certification (AHA or Red Cross BLS) is required.
• Valid state-issued driver's license and transportation required.

Responsibilities

• Manage the overall plan of care for hospice patients with the IDG.
• Assess and identify patient/caregiver problems; establish nursing goals and interventions.
• Collaborate with physician to establish medical plan of care for the patient.

Job Summary

Manage the over-all plan of care for a case load of hospice patients in collaboration with other members of the interdisciplinary group (IDG). Assess and identify patient/caregiver problems, establish nursing goals, and interventions. Work with physician to establish medical plan of care for the patient. Establish and maintain therapeutic relationship with patient and caregiver, including providing emotional support and teaching. Work closely with other members of the interdisciplinary group and communicates effectively both verbally and in writing with team members, patients, caregivers, physicians, pharmacists, and other health care professionals. Document the outcome of care. Provides dietary counseling for patient care needs as indicated. Assume on call responsibilities as scheduled.

Requirements

Education/Experience

• Graduate of an accredited School of Nursing
• Must be currently licensed as an RN in North Carolina and in good standings with NCBON.
• Experience in an acute, hospice, home health setting, or long-term care setting a plus!
• CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.
• Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
