AuthoraCare Collective seeks a Hospice RN Home Care Case Manager for the Burlington service area. The position is full-time (32 hours per week / 4 days) with CPR/BLS certification required.

The RN will manage care plans, collaborate with the IDG, and provide compassionate, patient-centered hospice care. Responsibilities include assessing needs, establishing goals and medical plans with physicians, and maintaining strong communication with patients, caregivers, and healthcare professionals.

Formación

• Graduate of an accredited School of Nursing.
• Must be licensed as an RN in North Carolina and in good standing with NCBON.
• CPR/BLS certification required.

Responsabilidades

• Manage the overall plan of care for hospice patients in collaboration with the IDG.
• Assess patient and caregiver needs, establish nursing goals and interventions.
• Coordinate with physicians to establish the medical plan of care.
• Maintain therapeutic relationships with patients and caregivers, providing emotional support and education.
• Document outcomes of care and participate in on-call duties.

AuthoraCare Collective is currently seeking a Hospice RN Hospice Home Care Case Manager for our Burlington service area. This position is full-time (32 hours per week / 4 days). CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.

Job Summary

Manage the over-all plan of care for a case load of hospice patients in collaboration with other members of the interdisciplinary group (IDG) . Assess and identify patient/caregiver problems, establish nursing goals, and interventions. Work with physician to establish medical plan of care for the patient.

Establish and maintain therapeutic relationship with patient and caregiver, including providing emotional support and teaching. Work closely with other members of the interdisciplinary group and communicates effectively both verbally and in writing with team members, patients, caregivers, physicians, pharmacists, and other health care professionals. Document the outcome of care. Provides dietary counseling for patient care needs as indicated. Assume on call responsibilities as scheduled.

Requirements

Education/Experience

• Graduate of an accredited School of Nursing
• Must be currently licensed as an RN in North Carolina and in good standings with NCBON.
• Experience in an acute, hospice, home health setting, or long-term care setting a plus!

CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.

Other

Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
