Job summary

Gilchrist Hospice Inova is seeking an experienced RN Case Manager to provide expert nursing and supportive care to terminally ill patients and their families in the Northern Virginia region. This role reports to the Team Manager and requires collaboration with the interdisciplinary team to identify needs and deliver comprehensive home or facility-based hospice care.

Ideal candidates hold a Diploma/ADN/BSN, Virginia RN licensure, and at least two years of nursing experience with strong

• Two years of nursing experience, or equivalent combination of education and experience.
• Knowledge of universal precautions and safe nursing practices.
• Excellent communication and interpersonal skills.

Responsibilities

• Complete initial assessment of patient and family to identify home care needs.
• Develop and implement the care plan in collaboration with the Interdisciplinary Group.
• Provide direct nursing care to patients in accordance with physician orders.
• Plan and supervise delivery of care by Home Health Aide (HHA).
• Update primary physician and interdisciplinary team regularly and document care in the chart.
