Job summary

Gilchrist Hospice in collaboration with Inova is seeking an experienced RN Case Manager to join our Northern Virginia team. You will identify patient and family needs and provide skilled nursing and supportive care for terminally ill patients and their families.

Requirements include a Diploma/ADN/BSN, Virginia RN licensure, and at least two years of nursing experience. Collaboration with the interdisciplinary team is essential to deliver compassionate, patient-centered care.

Qualifications

• Diploma, ADN and/or BSN from an accredited institution.
• Virginia RN license in good standing.
• Two years of nursing experience or equivalent.

Responsibilities

• Complete initial assessments of patient and family home care needs.
• Develop and implement the nursing care plan with the team.
• Provide direct nursing care in collaboration with the physician.
• Coordinate care delivery by home health aides.
• Educate family and facility staff on care skills and meds.
• Maintain charting and communicate updates to the team.
