Job summary

TriHealth is seeking a Continuous Care Licensed Practical Nurse (LPN) to provide direct patient care for hospice patients during acute crises. Care is delivered in patients' homes and community settings, with travel to various facilities covered by mileage reimbursement.

Night shift (7pm-7:30am) and every third weekend inclusion are required. This role offers shift differentials and crisis care incentives, along with TriHealth's comprehensive benefits package.

Qualifications

• Graduate of an approved nursing program.
• Current BLS for Healthcare Providers.
• Licensed Practical Nurse (NAPNES if prior to 1988).
• RN licensure acceptable per policy.

Responsibilities

• Collects data for nursing assessment and documents observations.
• Communicates findings to RN and health team members.
• Contributes to plan of care and implements it per scope of practice.
• Administers medications within licensure and safety standards.
• Maintains patient rights, dignity, and a safe therapeutic environment.
• Documents plan of care and patient response in a timely manner.
• Participates in discharge planning and goal development.
• Delegates tasks within scope and collaborates with physicians and caregivers.
