Job summary

The Care Team Hospice is seeking a compassionate Licensed Practical Nurse (LPN) to join our growing team in New Philadelphia, OH. This nights position offers 10-12 hour shifts, 3-4 nights per week, and the chance to deliver skilled nursing care and meaningful support to patients and families.

You will work with an experienced interdisciplinary team to monitor patient needs, document care in the EMR, and provide comfort and dignity during the hospice journey.

Qualifications

• Active LPN license required.
• Hospice/home health/long-term care experience preferred.
• Strong communication and patient care skills.

Responsibilities

• Provide skilled nursing care under RN direction per patient plan of care.
• Monitor patient conditions and communicate updates to the care team.
• Assist with symptom management, medications, treatments, and comfort measures.
