Agrace, a nationally recognized nonprofit hospice and palliative care provider in Wisconsin, is seeking a compassionate CNA for Homecare to support Portage, Pardeeville, Poynette and WI Dells area. This full-time role offers a 40-hour workweek, day shift, Monday-Friday, 8:00am-4:30pm, with no weekend or holiday rotations (except 3/year).

You will assist with activities of daily living, light homemaking, and follow the care plan, documenting care and communicating changes.

Qualifications

• High school diploma or GED or equivalent.
• Graduation from an accredited Certified Nursing Assistant program.
• Current, unrestricted Nursing Assistant Certification in state of employment.
• One-year personal care experience, hospice, or home health preferred.
• Valid driver’s license, auto insurance, and access to a reliable vehicle.
• Basic Microsoft Office skills.
• Experience using electronic health records; Epic preferred.
• Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property.
• Ability to use proper body mechanics, safe patient handling, and standard precautions.

Responsabilités

• Assist Patients with Activities of Daily Living.
• Provide light homemaker assistance.
• Follow the plan of care and document care.
• Communicate changes in condition and document care in the electronic health record.
• Participate in meetings to plan and evaluate patient/family services.
• Precept new staff and students.
• Delegate tasks to volunteers as appropriate.
