Zusammenfassung

Agrace is seeking a skilled Certified Nursing Assistant for homecare in the Oregon, WI area. This 0.6 FTE (24 hours/week) day shift role runs Monday–Friday, 8:00am–4:30pm, with weekend and holiday rotations.

Starting pay is $18.50/hour based on experience, plus a $3,000 sign-on bonus. You will provide patient- and family-centered care, document in the EHR, and support activities of daily living.

Qualifikationen

• High school diploma or GED or equivalent.
• Graduation from an accredited CNA program.
• Current unrestricted CNA certification in the state of employment.
• One year of personal care, hospice, or home health preferred.
• Valid driver's license and access to a reliable vehicle.
• Experience with electronic health records.

Aufgaben

• Assist with activities of daily living and light homemaker tasks.
• Follow the plan of care and document care needs.
• Read, understand, and document changes in care in the EHR.
• Communicate significant changes to the Interdisciplinary Team.
• Precept new staff and volunteers; participate in team meetings.
