Job summary

Bluegrass Care Navigators is seeking a Hospice RN Case Manager to deliver compassionate, comprehensive care to patients in home settings and other interim living arrangements. The role emphasizes active listening, holistic planning, and advocacy to support patients and families through hospice care.

The RN will coordinate interdisciplinary care, manage symptoms, and ensure high-quality patient-centered service with empathy and professionalism during challenging times.

Qualifications

• Graduate of an accredited school of registered nursing.
• Currently licensed as a Registered Nurse through the Kentucky Board of Nursing or through a participating Compact State Board of Nursing.
• Minimum of one year of general nursing experience preferred; new graduate nurses will be considered.
• Demonstrates strong clinical nursing skills and the ability to maintain professionalism during emotionally challenging situations.
• Provides compassionate, patient-centered care while serving diverse populations with respect and cultural sensitivity.
• Adheres to accepted professional, ethical, and legal standards of nursing practice.
• Demonstrates understanding of and commitment to hospice philosophy and the principles of death and dying.
• Maintains current CPR certification.
• Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements.

Responsibilities

• Delivers compassionate and comprehensive health care services to Hospice patients in a home care setting.
• Creates and implements a holistic plan of care based on the patient’s goals and preferences.
• Serves as the patient advocate to support, guide, and coordinate care for patients, families, and caregivers.
• Manages symptoms through pharmacologic and non-pharmacologic interventions for optimal comfort, functioning, and safety.
• Performs frequent monitoring and follow-up to evaluate treatments and plans of care.
• Maintains strong communication with an interdisciplinary team and works autonomously with professionalism.
