Job summary

Hospice of Florida in Jacksonville seeks a self-driven Community Liaison to build relationships with physicians and healthcare partners, advocating for our hospice services and delivering compassionate outreach.

You will drive referral growth across the Duval County service area, support patients and families during critical moments, and collaborate in a mission-driven, growth-focused team with a comprehensive benefits package.

Qualifications

• Bachelor’s degree preferred; related experience may substitute.
• Minimum two years of healthcare sales/marketing.
• Hospice/palliative marketing experience preferred.
• Ability to build relationships with physicians and staff.
• Must have reliable transportation and license; background screening required.

Responsibilities

• Establish and nurture relationships with key decision-makers in the medical community.
• Serve as an advocate for hospice services through regular outreach.
• Grow presence by positioning Hospice of Florida as the provider of choice in the service area.
• Drive referral growth through visits with physicians, discharge planners, SNFs, social workers, and others.
• Support patients and families during critical moments, guiding conversations and consent with empathy.
