Job summary

Addus HomeCare's Alamo Hospice unit in San Antonio, TX seeks a driven Hospice Liaison to grow referrals and census. You will partner with hospitals, clinics, and physicians, presenting hospice services and coordinating patient intake and transitions to home care.

Qualifications

• Minimum of one year of professional sales experience preferred.
• Experience in hospice, home health, or healthcare marketing a plus.
• Demonstrates good verbal and written communication and organization skills.

Responsibilities

• Coordinates and facilitates referrals to Hospice care based on admission criteria.
• Assist in information gathering, education, and form completion for patient intake and transition to home.
• Develop consultative relationships in assigned accounts by making effective customer contacts and presentations.
• Develop and implement marketing and education programs to increase awareness of company.
