Responsibilities

• Represent Vitality Hospice in the community through in-person visits and outreach.
• Develop and maintain relationships with senior living communities, hospitals, physician offices, and other healthcare partners.
• Manage a portfolio of assigned accounts with relationship-based engagement.
• Maintain follow-up with active referral sources.
• Communicate with discharge planners, case managers, physicians, and partners per HIPAA guidelines.
• Deliver educational presentations and luncheons to referral partners.
• Document visits and outcomes in the CRM.
