Job summary

Elara Caring is seeking a Hospice Bereavement Coordinator to lead grief counseling and bereavement services for patients, families, and staff. You will coordinate assessment and delivery of care, provide spiritual support, and help plan program development within a compassionate home-based hospice setting.

The role requires a bachelor’s degree in a related field and at least 1 year of hospice counseling experience, with travel up to 50% and strong organizational and communication skills.

Qualifications

• Bachelor's degree in psychology, social work, sociology, or related fields.
• 1 year of counseling experience in home health or hospice environment.
• Experience in family therapy regarding loss, grief, death and dying.
• Excellent organization, analytical, decision-making and communication skills.

Responsibilities

• Coordinates the assessment and delivery of grief counseling needs and services to ensure timely and appropriate services for family and caregivers.
• Provides spiritual support and/or counsel to patients, families, and staff.
• Plans bereavement program development, expansion, and refinement through annual evaluation of services.
• Provides bereavement support to hospice personnel coping with work-related grief through counseling and supervision of interns.
• Assists in evaluating and planning services to meet hospice personnel support.
• Functions independently but collaborates with the interdisciplinary team and clergy as needed.
