What you’ll bring as Hospice Bereavement Coordinator

Coordinates the assessment and delivery of grief counseling needs and services (one-to-one, groups, and ongoing follow-up) to ensure timely and appropriate services are provided to family and caregivers.
Provides spiritual support and/or counsel to patients, families, and staff consistent with patient and families’ belief systems.
Plans bereavement program development, expansion, and refinement through annual evaluation of services.
Provides bereavement support to hospice personnel coping with work-related grief through one-to-one counseling, and supervision of interns providing one-to-one counseling.
Assists in evaluating and planning services to meet hospice personnel support.
Functions independently but seek and accepts guidance from other members of the interdisciplinary group, or from a member of the community.
Contacts clergy or appropriate representatives of patients or families as indicated.

Qualifications

What is Required?

Bachelor’s degree in psychology, social work, sociology, or related fields; or 8 years’ relevant experience.
1 year of relevant experience in a counselor role in a Home Health service or Hospice environment is required
Experience in family therapy regarding loss, grief, death and dying.
Excellent organization, analytical, decision-making and communication skills
Able and willing to travel up to 50% as needed to support business and patient needs.
Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
Valid driver’s license and insurance and reliable transportation to perform job tasks

You will report to the Clinical Team Manager.
