Job summary

Qualifications

• Initial assessment of patient and family to determine hospice care needs and medical history.
• Provide grief counseling services (one-to-one, group, and follow-up) for families.
• Plan bereavement program development and annual service evaluations.
• Coordinate bereavement support for hospice staff and supervise interns.
• Provide bereavement information and referrals to community resources.
• Assist in evaluating and planning services to meet staff support needs.
• Work independently while coordinating with the Interdisciplinary Group and community partners.
• Participate in in-service education, staff meetings, and committees.
• Monitor changes in patients’ condition and report to the IDG; advance knowledge.
• Other duties as assigned.

Responsibilities

• Coordinate grief support services and ensure timely delivery.
• Lead bereavement program development and evaluation.
• Support hospice staff through bereavement resources.
• Maintain community resource referrals.
• Collaborate with IDG and community partners.
• Participate in in-service education and committee work.
• Document patient and family status and report to IDG.
• Uphold CPR certification and professional standards.
• Handle other duties as assigned.
