Job summary

Affinity Hospice Management LLC is seeking a Bereavement Coordinator to plan and maintain a one-year bereavement program for families and caregivers following a patient’s death.

The role involves coordinating grief counseling, community referrals, and program evaluation, while supporting hospice staff and interns. A related degree and clinical grief counseling experience are required or an MSW, with hospice exposure preferred.

Qualifications

• Bachelor’s degree or equivalent theological degree with experience with families/caregivers and patients through death/dying issues.
• Master’s degree in Social Work with qualifications of experienced clinical grief counseling.
• Previous hospice experience preferred.

Responsibilities

• Coordinate assessment and delivery of grief counseling needs and services to family/caregivers.
• Plan Bereavement Program development, expansion and evaluation.
• Provide bereavement support to hospice personnel and interns.
• Provide bereavement information and referral services to community callers.
• Assist in evaluating and planning services to meet hospice personnel support needs.
• Function independently but seek guidance from interdisciplinary team.
• Participate in hospice activities and committees.
• Engage in quality assessment and performance improvement.
• As a mandatory reporter report to Dept. of Health within 14 days.
• Report abandonment, abuse, financial exploitation, or neglect as required.
• Participate in continued professional development.
