Job summary

Covenant Living and CovenantCare Home Health and Hospice seeks a Bereavement Coordinator to plan, implement, and maintain a bereavement program for families and caregivers for up to one year after a patient's death.

The role coordinates assessments and delivers grief counseling in one-to-one and group settings, while expanding community resource lists and providing information to callers. Collaboration with hospice teams and interns is expected.

Qualifications

• Understanding of grief response and counseling individuals or groups
• Ability to work as part of an interdisciplinary team
• Good verbal and written communication and organization skills

Responsibilities

• Plan, implement, and maintain a bereavement program for families/caregivers up to one year after death
• Coordinate assessment and delivery of grief counseling (one-to-one, groups, follow-up)
• Provide bereavement information and referrals to the community
• Participate in hospice education, meetings, and committees
• Support hospice personnel coping with work-related grief and supervise interns
