Job summary

VIA Health Partners is seeking a licensed professional to provide grief care and counseling within our hospice program. The role entails individual and group counseling, coordinating with the interdisciplinary team, and developing care plans based on survivor assessments and psychosocial evaluations.

The ideal candidate has a Master’s degree in Social Work, Counseling, Divinity or a related field, with clinical concentration and hospice or bereavement experience.

Responsibilities

• Provide grief care through individual and group counseling.
• Develop, implement, and evaluate plan of care for grief and Bereavement.
• Coordinate with interdisciplinary team to support care goals.
• Conduct community education and presentations on grief and loss.
