Frontpoint Health is seeking a Hospice Registered Nurse (RN) for a full-time on-call role. The position involves traveling to multiple homes daily to provide skilled nursing care, including IV administration, wound care, and palliative support. The schedule follows a 7 days on / 7 days off rotation.

The ideal candidate holds a Texas RN license, has at least 3 years Hospice experience, CPR certification, and reliable transportation to cover assigned home visits across Texas.

Qualifications

Responsibilities

• Provide skilled nursing care per the plan of care (medication administration, wound care, dressing changes).
• Perform initial admission assessments and ongoing patient evaluations.
• Maintain visit notes within 24 hours and complete OASIS within 48 hours.
• Maintain comprehensive clinical records and medication profiles.
• Educate patients and families on home health procedures and strategies.
• Maintain patient PHI confidentiality per HIPAA.

Hospice - Full Time On-Call - Registered Nurse (RN)

Job Summary:

As a Hospice RN, you will be responsible for traveling to several homes a day to administer healthcare services and help patients maintain their independence. Duties may include administering at home IVs, changing dressings, cleaning wounds, and foley/catheter insertions. This is a Full-Time On-Call RN position. The work schedule will follow a 7 days on / 7 days off rotation.

Responsibilities:

• Case management and supervised visits.
• Perform skilled and comprehensive assessment of the patient during the initial admission.
• Assess and chart observations of the patient’s condition at each visit (vital signs.
• Provide skilled nursing care as needed in accordance with the plan of care (administering medication, cleaning/dressing wounds, assessing the healing progress).
• Provide proper patient education for home health care procedures and strategies.
• Responsible for the overall clinical record maintenance to include visit note (must be completed within 24 hours), comprehensive assessments, including OASIS (must be completed within 48 hours), medication profile, orders/plan submitted in a timely manner, etc.
• Maintain confidentiality of patient PHI through HIPPA law.
• Keep the patient comfortable through palliative care
