Job summary

Community Healthcare of Texas is seeking an On-Call Registered Nurse to provide emergency support and skilled visits during non-standard hours. The role emphasizes compassionate hospice and palliative care, with on-call coverage to ensure patient safety and continuity of care.

The RN will document care, participate in team conferences, and communicate with families and the care team, maintaining professional standards and adherence to Medicare participation requirements.

Qualifications

• Registered Nurse in Texas with active license.
• Valid driver's license and reliable transportation.
• Minimum 2 years RN experience.
• 6+ months in an On-Call role.
• Crisis management experience preferred.
• Hospice or Home Health experience preferred.
• Proficient with Microsoft Office.

Responsibilities

• Provide emergency on-call visits and nursing care per plan; document per Medicare requirements.
• Support hospice patients during non-office hours and via phone as needed.
• Attend weekly team conferences and contribute necessary information.

Community Healthcare of Texas has cared for patients with serious and terminal illnesses throughout North Central Texas. Community Healthcare of Texas is currently recruiting for a Registered Nurse (RN) On Call.

POSITION SUMMARY

The On-Call RN provides emergency support and skilled visits to patients on an on-call basis during non-standard work hours.

ESSENTIAL FUNCTIONS

• Make emergency on-call visits demonstrating the skills, knowledge, and judgment necessary to implement the nursing care plan; document appropriately to meet Medicare conditions of participation; provide quality care through symptom management and emotional support.
• Participates in weekly team conferences by contributing necessary information and documentation.
• Responsible for Hospice patients during non-office hours, providing a feeling of security through phone communication and skilled visits as indicated.
• Assumes responsibility for attendance, dependability, and personal appearance with minimal supervision.
• Must be capable of handling interpersonal relationships, able to communicate effectively, and demonstrate professional development.
• Must have the ability to use, store, and care for equipment, use safety in work habits, and respond properly in crises.
• May be required to evaluate appropriateness for hospice care and admit patients.

QUALIFICATIONS

• Registered Nurse in the state of Texas
• Hold a valid driver’s license and have a dependable means of transportation
• Minimum 2 years of RN experience required
• 6+ months in an On-Call role required
• Crisis management experience strongly preferred
• 1 year Hospice or Home Health experience strongly preferred
• Proficient computer skills are required; Microsoft Office suite experience preferred.
