Job summary

Four Pillars Hospice is seeking a Registered Nurse (RN) to provide nursing care to hospice patients in their homes and coordinate care with the Interdisciplinary Team (IDT), physicians, patients, families, and referral sources. The RN assesses patient needs and implements the plan of care to ensure quality end-of-life care.

Responsibilities include initial and ongoing patient assessments, developing and updating individualized plans of care, coordinating with IDT and families, educating patients

Qualifications

  • Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing.
  • Current RN license in the state of practice and/or compact license eligibility.
  • Must have hospice experience.
  • Understanding of hospice regulatory requirements, documentation standards, and compliance guidelines.
  • Strong clinical, communication, and interpersonal skills.
  • Knowledge of end-of-life and palliative care.
  • Ability to read, write, and communicate effectively in English.

  • Practice infection control and standard precautions.
  • Participate in QAPI and hospice in-service education programs.
  • Provide on-call services as assigned.
  • Promote hospice philosophy and quality patient care.

Position Summary

The Registered Nurse (RN) provides nursing care to hospice patients in their place of residence and coordinates care with the Interdisciplinary Team (IDT), physicians, patients, families, and referral sources. The RN is responsible for assessing patient needs, implementing the plan of care, and ensuring quality end-of-life care.

  • Valid auto liability insurance

  • Flexible schedule, local travel, and on-call responsibilities required

  • Ability to stand, walk, lift up to 50 pounds, and assist with patient transfers
