Four Pillars Hospice in Texas is seeking a Registered Nurse (RN) to provide compassionate end-of-life care in patients' homes and coordinate with the IDT, physicians, families, and referral sources.
You will assess needs, implement care plans, educate patients and caregivers, supervise staff, document findings, and participate in QAPI and in-service programs while maintaining infection control and regulatory compliance.
The Registered Nurse (RN) provides nursing care to hospice patients in their place of residence and coordinates care with the Interdisciplinary Team (IDT), physicians, patients, families, and referral sources. The RN is responsible for assessing patient needs, implementing the plan of care, and ensuring quality end-of-life care.
Perform initial and ongoing patient assessments
Develop, implement, and update individualized plans of care
Coordinate care with the IDT, physicians, patients, and families
Provide patient and caregiver education regarding disease process, medications, end-of-life care, and safety
Determine visit frequency based on patient needs and acuity
Supervise Hospice Aides and LVN/LPN staff
Document clinical findings, interventions, and patient progress accurately
Report changes in patient condition to physicians and team members
Practice infection control and standard precautions
Participate in QAPI and hospice in-service education programs
Provide on-call services as assigned
Promote hospice philosophy and quality patient care
Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing
Current RN license in the state of practice and/or compact license eligibility
Must have hospice experience
Understanding of hospice regulatory requirements, documentation standards, and compliance guidelines
Strong clinical, communication, and interpersonal skills
Knowledge of end-of-life and palliative care
Ability to read, write, and communicate effectively in English
Current driver’s license
Reliable transportation
Valid auto liability insurance
Travel to patient homes required
Possible exposure to blood, bodily fluids, and infectious diseases
Flexible schedule, local travel, and on-call responsibilities required
Ability to stand, walk, lift up to 50 pounds, and assist with patient transfers
