Job summary

Choice Hospice, Inc. is seeking a Hospice Field RN PRN to provide patient-centered nursing care in homes and community settings.

The role focuses on comprehensive assessments, care planning, and coordination with interdisciplinary teams to ensure comfort and dignity. The RN will conduct admissions assessments, implement and document care plans, and educate patients and families about disease progression and end-of-life care.

Qualifications

• Current RN license is required.
• CPR certification is required.
• Valid driver’s license is required.
• Valid auto insurance is required.
• At least 2 years Hospice RN experience preferred.
• HomeCare HomeBase experience preferred.

Responsibilities

• Conduct initial and ongoing comprehensive assessments of patients in home or other care settings.
• Apply criteria for admission and re-certification to hospice care.
• Develop, implement, document, and revise the plan of care to ensure quality and continuity of care.
• Educate patients and families about the disease process, self-care, and end-of-life care; coordinate with IDG members.
• Determine scope and frequency of services based on patient/family needs and acuity.
• Provide clinical direction to Hospice Aide and LPN/LVN to ensure quality care.
• Prepare progress notes showing progress toward goals and maintain thorough documentation.
• Inform physicians and staff of changes in patient needs and responses to care; participate in on-call duties.
