Job summary
Hospice of Southern Illinois in Belleville, IL is recruiting a Staff Registered Nurse/Case Manager to assess patient needs, plan and coordinate care, and evaluate outcomes within an Interdisciplinary Team. The role requires an Illinois nursing license, at least 1 year hospice experience preferred, and a valid driver's license with reliable transportation.
Pay is $39.21 per hour, full-time, with a Monday–Friday schedule and on-call rotation.
Qualifications
- Graduate of State-approved nursing program.
- Associate Degree in Nursing or Diploma in Nursing required.
- Current Illinois nursing license.
- Certification in hospice and palliative care preferred.
- CPR certification preferred.
- One year hospice experience preferred.
Responsibilities
- Maintain a caseload and oversee patient care with the IDT.
- Provide direct patient care and participate in on-call after hours.
- Perform assessments, medication reviews, and document care plans.
- Coordinate with IDT and supervise LPNs and HA plan of care updates.
