Job summary

Joliet Area Community Hospice in Joliet, IL is seeking an RN Field Case Manager to join its interdisciplinary team and deliver skilled nursing assessments and hospice services in patients' homes and communities.

You will coordinate care plans, administer treatments, educate patients and families, and participate in on-call coverage across the service area, Monday–Friday, with travel to DuPage, Will, and Southern Cook counties as needed.

Qualifications

• Current RN license.
• Graduate of accredited nursing program.
• Minimum of 2 years of nursing experience, with at least 1 year in public health, home care, or hospice preferred.
• Valid driver’s license and insured, reliable vehicle.

Responsibilities

• Deliver skilled nursing care and hospice services in home and community settings.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Administer medications and treatments while monitoring patient symptoms and outcomes.
• Educate patients and caregivers on care, self-management, and disease progression.
• Collaborate with the interdisciplinary team to ensure coordinated, high-quality care.
• Maintain accurate documentation and participate in on-call coverage as needed.
• Coverage may include portion of DuPage, Will and Southern Cook Counties (Bolingbrook, Romeoville & Surrounding Areas).
• Monday–Friday schedule; minimum of 20 on-call hours per month (may increase).

Education

RN license

Graduate of accredited nursing program

Minimum of 2 years nursing experience; 1 year in public health, home care, or hospice preferred
