Job summary

Houston Hospice is seeking a Full-Time Pediatric RN Field Case Manager in Houston, TX. The role focuses on overall management of pediatric patient cases, coordinating care with clients, families, and multiple providers to ensure comprehensive, compassionate end-of-life care.

The position requires collaboration with RNs, LPNs, social workers, and other team members, with duties including care planning, documentation, and home visits to optimize outcomes for children and their families.

Qualifications

  • Must hold an active RN license (and/or LPN depending on role) and meet state requirements.
  • 2+ years of pediatric or case management experience preferred.
  • Experience coordinating care with physicians, nurses, social workers, and other providers.
  • Experience documenting case notes and maintaining compliance with state regulations.
  • Ability to develop and maintain individualized care plans based on clinical assessments and family needs.

Responsibilities

  • Coordinate care with providers including RNs, LPNs/LVNs, and other clinicians.
  • Maintain case documentation and notes per state regulations.
  • Develop and maintain care plans for each client based on assessments and family needs.
  • Coordinate care with other providers as necessary.

Summary:

We are seeking a candidate who would be responsible for the overall management of pediatric patient's cases. Responsibilities include coordination of care with the client, family, and other providers.

Duties and Responsibilities:

  • Coordinate care with providers, including: Registered Nurses (RN), Licensed Practical Nurses (LPN), and Licensed Practical Nurses (LPN/LVN)
  • Maintain case documentation in accordance with state regulations.
  • Maintain case notes in accordance with state regulations.
  • Develop and maintain care plans for each client on the basis of clinical assessment and family needs.
  • Coordinate care with other providers as necessary
  • Maintain appropriate communication with the admitting physician or psychiatrist regarding progress reports, changes in status, and as necessary, refer patients to other providers or services.
  • Conduct home visits as necessary to ensure optimal outcomes.
  • Review patient charts weekly to ensure appropriate follow-up and referral as necessary.
  • Ensure that appropriate follow-up is provided by all other providers as necessary.
  • Coordinate care with community agencies as necessary to ensure appropriate follow-up.
  • Provide training to staff on client management and referral processes.
  • Develop, modify, and maintain case files as needed in accordance with state regulations.
  • Adhere to all policies and procedures of the agency or organization that employs you.

Experience: Registered Nurse: 2 years (Preferred) Licensed Practical Nurse: 2 years (Preferred) Case Managers: 2 years (Preferred)
