Job summary

Brighton Hospice in Oregon is seeking a compassionate Registered Nurse (RN) to provide comprehensive hospice care across patients' homes, SNFs, and assisted living facilities.

You will be part of an interdisciplinary team, delivering psychosocial assessments and care planning while maintaining high-quality patient care. The RN will serve in Case Management, Admissions, or Resource roles, with required driving to cover the service area.

Qualifications

• Current RN licensure in good standing.
• Works well within a team with nurses, social workers, chaplains, and CNA leadership.
• Able to work 1-1 with patients and independently with daily activities.

Responsibilities

• Provide care for a group of patients visiting them at their homes or facilities.
• Contribute to interdisciplinary team with psychosocial assessments and care planning.
• Work in Case Management, Admissions, or Resource role.
