Job summary

Hospice of the East Bay is seeking a Healthcare Liaison to educate physicians, hospitals, and care teams about hospice and palliative services in Alameda and Contra Costa County. This role collaborates with clinicians to promote awareness and coordinate outreach efforts.

Ideal candidates have at least 2 years in hospice or home care, strong presentation skills, and a valid California driver’s license. Join a mission-driven nonprofit committed to compassionate care and community partnership.

Qualifications

• Minimum of 2 years of hospice/home care experience.
• Strong clinical knowledge and critical thinking to assess and plan referrals.
• Highly organized self-starter with initiative and flexibility.
• Excellent verbal, written, and public presentation skills.
• Knowledge of healthcare systems, Medicare/Medi-Cal and Hospice benefits.
• Ability to work independently, manage multiple priorities, and adapt to changing needs.

Responsibilities

• Serve as an educational resource on hospice and palliative care for physicians, hospitals, and clinic staff.
• Define and interpret agency services and policies to hospital personnel, physicians, patients/families.
• Plan and participate in educational presentations, community events, and network meetings.
• Maintain regular contact with referral sources including case managers and physicians.
• Act as liaison for Residential Care/Assisted Living Facilities and assist with waiver requests.
• Maintain records of activities and produce timely reports.
• Participate in quality improvement activities and agency meetings.
