Job summary

Three Oaks Hospice is seeking a dedicated Dietitian to provide nutritional assessments, counseling and interventions for terminally ill patients and families within our hospice program.

As an active member of the Interdisciplinary Group (IDG), you will help develop and implement individualized Plans of Care and work with the care team to support comfort and quality of life for patients receiving hospice services.

Qualifications

• Bachelor’s Degree in nutrition or related subjects, preferred.
• Registered or Licensed Dietitian as required by the state where practicing; or eligible for registration.
• Must have current driver’s license.
• One year experience as a dietitian; hospice or terminally ill patient experience preferred.
• Knowledge of nutritional needs of terminally ill patients; good interpersonal skills; EMR proficiency; hospice philosophy understanding.
• Reliable transportation and valid auto liability insurance.

Responsibilities

• Provide nutritional assessments, dietary counseling, education and interventions for terminally ill patients and families.
• Active participant in the Agency's Interdisciplinary Group (IDG) and contribute to Plan of Care (POC) development and implementation.
