Job summary

Sage Hospice Primary & Palliative Care is seeking a Dietitian to perform nutritional assessments, counseling, education and interventions for terminally ill patients and families. The Dietitian participates in the Agency's Interdisciplinary Group (IDG) and contributes to developing and implementing an individualized Plan of Care.

Candidates should have a nutrition degree, RD/LD or eligibility, and a valid driver’s license. Hospice experience is preferred and reliable transportation is required.

Qualifications

• Bachelor’s Degree in nutrition or related subjects.
• Registered or Licensed Dietitian as required by the state; or eligible.
• Must have a current driver’s license.
• One year experience as a dietitian; hospice experience preferred.
• Knowledge of terminally ill nutrition; good interpersonal skills; EMR experience.
• Reliable transportation and auto liability insurance required.

Responsibilities

• Provide nutritional assessments, dietary counseling, education and interventions to terminally ill patients and families.
• Active participant in the IDG and assist in developing, coordinating, and implementing an individualized Plan of Care (POC).

The Dietitian is an active participant in the Agency's Interdisciplinary Group (IDG) and assists in developing, coordinating, and implementing an individualized Plan of Care (POC).

Qualifications:

• Bachelor’s Degree in nutrition or related subjects, preferred.
• Registered or Licensed Dietitian as required by the state where practicing; or is eligible for registration by the American Dietetic Association or has documented equivalency in education or training.
• Must have current driver’s license.
• One (1) year experience as a dietician/dietitian. Experience in hospice or working with terminally ill patients preferred.
• Knowledge of nutritional needs of the terminally ill patient. Good interpersonal skills. Demonstrates ability to work with electronic medical records. Good understanding of hospice philosophy.
• Reliable transportation and valid current auto liability insurance required.
