Job summary

Sage Hospice Primary & Palliative Care in Scottsdale, AZ is seeking a Dietitian to provide nutritional assessments, counseling, education, and interventions to terminally ill patients and their families. The Dietitian will participate in the Agency's Interdisciplinary Group (IDG) and help develop and implement an individualized Plan of Care.

Qualifications

• Bachelor’s Degree in nutrition or related subjects (preferred).
• Registered or Licensed Dietitian as required by the state; or eligible for registration by the American Dietetic Association or has documented equivalency.
• Must have current driver’s license.
• One year experience as a dietician/dietitian; hospice experience preferred.
• Knowledge of nutritional needs of the terminally ill patient; good interpersonal skills; familiarity with EMRs; understands hospice philosophy.
• Reliable transportation and valid auto liability insurance required.

Responsibilities

• The Dietitian will provide nutritional assessments, dietary counseling, education and interventions to the terminally ill patient and their family and/or care givers.
• The Dietitian is an active participant in the Agency's Interdisciplinary Group (IDG) and assists in developing, coordinating, and implementing an individualized Plan of Care (POC).
