Three Oaks Hospice is seeking a dedicated Dietitian to provide nutritional assessments, counseling and interventions for terminally ill patients and families within our hospice program.
As an active member of the Interdisciplinary Group (IDG), you will help develop and implement individualized Plans of Care and work with the care team to support comfort and quality of life for patients receiving hospice services.

The Dietitian will provide nutritional assessments, dietary counseling, education and interventions to the terminally ill patient and their family and/or care givers. The Dietitian is an active participant in the Agency's Interdisciplinary Group (IDG) and assists in developing, coordinating, and implementing an individualized Plan of Care (POC).
