Job summary
Hospice of the Midwest in Edina, MN is seeking a Community Liaison to be the face of our organization, promoting hospice services to healthcare professionals, community partners, and families. Qualifications
- Bachelor's degree in a related field required.
- 2+ years in healthcare marketing or hospice outreach preferred.
- Experience with CRM systems and referral networks is a plus.
- Valid state driver's license and auto insurance proof may be required.
- Strong communication and organizational skills are essential.
Responsibilities
- Visit hospitals, clinics, and senior facilities to promote hospice services.
- Build relationships with doctors, nurses, and referral partners.
- Attend community events to raise awareness and represent the organization.
- Track outreach activity in CRM and report on results.
- Educate families and professionals on hospice benefits and referrals.
- Coordinate with the care team to ensure smooth patient referrals.
