Hospice of the Midwest is seeking a Community Liaison in Monticello, MN to promote hospice services to healthcare professionals and families. You will build solid relationships with clinicians and referral partners, represent our brand at community events, and manage outreach activities in our CRM system.

Qualifications

• Bachelor's degree in a related field and sales/outreach experience.
• Advanced degree or certification in healthcare marketing preferred.
• Minimum of 2 years in healthcare marketing.

Responsibilities

• Visit hospitals, clinics, and senior care facilities to promote our hospice services.
• Build relationships with doctors, nurses, and referral partners.
• Attend community events to raise awareness and represent our brand.
• Track outreach activity in CRM and plan improvements.
• Educate families and professionals on hospice benefits.
• Ensure smooth patient referrals with care team.
• Complete reports and admin tasks on time.
• Follow healthcare rules and regulations.
