Job summary

Hospice of the Midwest in Edina, MN seeks a Community Liaison to promote hospice services to healthcare professionals, families, and partners. This full-time role involves building referral networks, attending events, and driving outreach in the community.

You will work with care teams to ensure smooth patient referrals, stay current on hospice care, and represent the organization with dignity and compassion. Strong communication and CRM skills are essential.

Qualifications

• Bachelor’s degree in a related field and prior experience in healthcare sales or hospice outreach required.
• Advanced degree or certification in healthcare marketing, and experience with CRM systems and referral networks preferred.
• Minimum of 2 years of healthcare marketing experience.
• State driver’s license required.
• Proof of auto insurance required.
• Strong interpersonal and communication skills.
• Proficiency in healthcare and administrative software applications.
• Analytical and problem-solving capabilities.
• Time management and organizational skills.
• Ability to work independently with minimal supervision.
• Collaborate professionally across organizational levels and communities served.
• Maintain the highest standards of customer service and professionalism.

Responsibilities

As a Community Liaison You will:

• Visit hospitals, clinics, and senior care facilities to promote our hospice services

• Build strong, ongoing relationships with doctors, nurses, and referral partners

• Attend community events to raise awareness and represent our brand

• Track and manage outreach activity in our system (CRM)

• Use market insights to plan and improve outreach efforts

• Educate families and professionals on hospice benefits and how we can help

• Work with our care team to ensure smooth patient referrals and quality care

• Stay current on hospice care through training and self-learning

• Complete reports, documentation, and other admin tasks on time

• Follow all healthcare rules and regulations

Qualifications

• Bachelor's degree in a related field and prior experience in healthcare sales or hospice outreach required

• Advanced degree or certification in healthcare marketing, and experience working with CRM systems and referral networks preferred

• Minimum of 2 years of healthcare marketing experience.

• Possess a current and valid state driver’s license

• Provide proof of auto insurance

• Demonstrate strong interpersonal and communication skills

• Exhibit proficiency in healthcare and administrative software applications

• Apply advanced analytical and problem-solving capabilities

• Manage time and tasks effectively with strong organizational skills

• Work independently with minimal supervision

• Collaborate professionally with individuals at all organizational levels and within the communities served

• Uphold the highest standards of customer service and professionalism
