Crescent Hospice in Buffalo, NY seeks a self-driven Community Liaison to promote our hospice services and develop trusted relationships with referral sources and healthcare partners. You’ll champion our mission and help families access compassionate care, driving census growth in the Buffalo area.

The role focuses on outreach, physician visits, and community engagement, with a base salary plus uncapped commissions and a sign-on bonus.

Qualifications

• Bachelor’s degree preferred; higher education may be substituted by experience.
• Minimum 2 years in healthcare sales/marketing; hospice marketing preferred.
• Ability to establish relationships with staff, marketing contracts, and clinical teams.
• Proven ability to develop relationships with physicians, healthcare professionals, and community agencies.
• Prior clinical experience is a plus.
• Willingness to drive with reliable transportation and valid license.

Responsabilités

• Establish and nurture relationships with physicians and healthcare partners to build trust within the medical community.
• Serve as an advocate for hospice services, conducting outreach aligned with compassionate, high-quality care.
• Grow presence by positioning Crescent Hospice as the provider of choice across the service area.
• Drive referral growth through regular visits with referring physicians, discharge planners, SNFs, social workers, and other professionals.
• Support patients and families by discussing hospice care, explaining services, and guiding consent with empathy.
