Job summary

Continuum Hospice in the San Jose area is seeking a self-driven Community Liaison (Hospice Marketer/Sales Representative) to build trust-based relationships with referral sources, physicians, and healthcare partners. You will drive census growth by raising awareness of hospice services and representing our compassionate care team.

Qualifications

• Bachelor’s degree preferred; experience may substitute for education.
• Minimum two years of healthcare sales/marketing experience; hospice/palliative marketing preferred.

Responsibilities

• Establish and nurture relationships with key decision-makers, including physicians and healthcare partners.
• Advocate for hospice services through regular outreach reflecting commitment to compassionate, high-quality care.
• Grow presence by positioning Continuum Hospice as the provider of choice in the service area to meet growth and census goals.
• Drive referral growth through regular visits with referring physicians, discharge planners, SNFs, social workers, and other professionals.
• Support patients and families during critical moments by leading conversations about hospice care with empathy and clarity.
