Job summary

Favoured Hospice is seeking a Hospice Liaison to build meaningful relationships with referral partners, educate families and providers about hospice services, and ensure compassionate, clear transitions for patients entering hospice care. You will collaborate with intake and clinical teams to facilitate timely assessments and admissions.

Qualifications

• Experience in hospice, home health, senior care, or healthcare marketing preferred.
• Training provided for hospice philosophy and eligibility.
• Exceptional communication, presentation, and relationship-building skills.
• Ability to work independently while staying connected to a supportive team.

Responsibilities

• Build and nurture strong relationships with hospitals, skilled nursing facilities, assisted living communities, clinics, and other referral partners.
• Educate patients, families, and healthcare professionals about hospice philosophy, eligibility, and Favoured Hospice services.
• Respond promptly to referral inquiries and collaborate with intake and clinical teams to ensure timely assessments and admissions.
• Participate in community outreach efforts to increase awareness of Favoured Hospice's mission and services.
• Maintain accurate documentation of outreach activities, referral trends, and community needs.
• Represent Favoured Hospice at community events, facility meetings, and professional gatherings with warmth and professionalism.
