Job summary
Residential Home Health and Residential Hospice is seeking a compassionate Certified Nursing Assistant to provide hospice care in patients' homes around Evanston, IL. This role offers autonomy, meaningful patient relationships, and opportunities for career growth.
The CNA will deliver hands-on care, assist with ADLs, and support families while documenting care in the EMR. A valid CNA certificate, High School diploma, and driver’s license are required; a sign-on bonus of $5,000 is available.
Qualifications
- Active CNA certificate licensed in the state you work.
- High School Diploma or equivalent.
- Minimum one year healthcare experience; hospice experience preferred.
- Valid driver's license and ability to drive ~20% of workday.
Responsibilities
- Deliver compassionate, patient-centered care in hospice settings.
- Assist with activities of daily living while monitoring changes in condition.
- Provide support to patients and caregivers by reinforcing care plans.
- Document care accurately in the EMR and report findings to the team.
- Work independently while collaborating with interdisciplinary team.
- Travel to patient homes within the assigned territory.
