Job summary

Residential Home Health and Residential Hospice is seeking a Hospice Certified Nursing Assistant (CNA) in Northbrook, IL to deliver compassionate, patient-centered care in patients' homes. You will work with an interdisciplinary team to support comfort, dignity, and quality of life, while traveling to patient locations within your territory.

The role requires an active CNA certificate, a high school diploma, and at least one year of healthcare experience.

Qualifications

• Active Certified Nursing Assistant (CNA) certificate in the state you work.
• High School Diploma or certificate.
• Minimum one year healthcare experience, Hospice experience preferred.
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life
• Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey
• Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory
