Job summary

Residential Home Health and Hospice in Naperville, IL is hiring a full-time Certified Nursing Assistant to deliver compassionate in-home hospice care. You will partner with an interdisciplinary team to promote comfort, dignity, and quality of life while assisting patients with daily activities and accurately documenting care in the EMR.

Active Certified Nursing Assistant (CNA) certificate in the state you work

Minimum one year healthcare experience
Valid driver’s license and ability to travel ~20% of workday to patient locations

Responsibilities

Deliver compassionate, patient-centered care in a home health/hospice setting
Assist with activities of daily living (ADLs)
Observe and report changes in condition to support safety and independence
Educate and support patients and caregivers by reinforcing care plans
Maintain accurate, timely documentation in the EMR
Travel to patient homes within assigned territory
